An accurate diagnosis for depression, bipolar disorder, anxiety disorders, other psychiatric disorders or alcohol or drug dependence can only be made by a physician or qualified mental health professional after a complete evaluation, including a physical exam, to rule out any other medical illnesses or conditions that may account for symptoms. This survey records non-individually identifiable scores and data for aggregate reporting only.
